NOTICE OF HEARING

(Rev. Code, Sec. 3107.11)

IN THE MATTER OF

THE ADOPTION OF:

Jacob Israel Chicko

Case No. 2022 AD 42

Jeffrey Walker, whose last known address is 417 Hall Street Northeast, Warren, Ohio 44483 is hereby notified that on the 4th day of October 2022, Jeffrey Thomas Chicko and Pamela Marie Chicko filed in Case Number 2022 AD 42 in this Court, a petition to adopt Jacoby Cleo Walker,  born on 30th day of April 2014, and change his name to Jacob Israel Chicko

The petition contains the allegation that Jeffrey Walker, natural/putative father of said child, has failed without justifiable cause to communicate with the minor or provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding the filing of adoption petition, the placement of the minor in the home of the petitioner(s).
